Hi First_Name,

You Have Been Enrolled in Your Insurance Plan

This email is to confirm that you have been enrolled in UW-Stout’s International Student Health Insurance Program. Complete information on the insurance policy is attached.

This is a required insurance plan for all international students except those sponsored by the following pre-approved programs:

Saudi Arabia Cultural Mission (SACM): SACM students must have a current Financial Guarantee letter on file in order to be waived from UW-Stout’s insurance. SACM STUDENTS: KEEP READING!

Continuing Students: Your current insurance plan ends on August 4, 2020. Your insurance coverage will not be disrupted as your new plan begins on August 5, 2020. Please download a new copy of your card and remember to carry it with you at all times. 

To create your account, take the following two steps: 

  1. Login or Create an account with Wellfleet: Create an account or login here
    • use your last name, Stout ID number and your date of birth (mm/dd/yyyy)
  2. Once in your account, click 'Student Options' and click 'Print ID Card(s)'

If later your lose your card, or require another copy, you can download a pdf here. You will need to login to your account to download your card. Students may download a new pdf as often as needed.

SACM Students: Please Submit Your Financial Guarantees!

SACM Students, if you are continuing your studies in the fall semester, please check the expiration date on your most recent Financial Guarantee.

If your guarantee expires BEFORE the end of fall semester, you must request an updated letter with an expiration dated December 22, 20220 or later.

Please email a copy of your Financial Guarantee  to globaled@uwstout.edu 
as soon as possible
. Please rememeber that there is a deadling with waiving insurance and failure to meet this deadline could result in you being charged for insurance in Fall 2020.
