This form is for reporting behavior(s) of a student organization which allegedly violates the UW-Stout Student Organization Code of Conduct.

Incident Reporting Form

This form is intended to be used report alleged organization violations of the UW-Stout Student Organization Code of Conduct.

If the report you are hoping to submit requires investigation by the Dean of Students office regarding individual student behaviors, a student of concern, hate bias, discrimination, or related issue you should fill out the Dean of Students Office Incident Reporting Form.

All reports filed here are routed to appropriate University personnel if necessary. Reports may be subject to mandatory reporting guidelines. If this is an emergency requiring an immediate response - please call 911.
